     Capitola-Soquel Little League Online Registration

 

You should have all scanned registration documents (see below) on your computer before starting this online registration process.
 (Online registrants do not have to upload Medical Release Forms or Volunteer Forms - our software will automatically generate those forms based on your online registration entries!)


Don't have a scanner? You can take digital photos of your registration material and upload those images! (Just make sure they are readable)


Required Registration Documents:

  •   Player's Birth Certificate (one for each sibling in CSLL)
  •   Parent's Driver's License
  •   Three (3) proofs of residency (two with Driver's License)
    •

    Note: You must provide copies of documents from THREE of the following categories to determine residency:
       (We recommend the '3 Autos': Driver's License, auto registration, auto insurance)

    Utility bills (noted below) only count as one document!

    • Utility bills (e.g. gas, electric, water/sewer, phone, heating, waste disposal)
    • Driver's License or ID Card (Required)
    • Voter's Registration
    • School records
    • Welfare/child care records
    • Federal records
    • State records
    • Local (municipal) records
    • Support payment records
    • Homeowner or tenant records
    • Financial records (loan, credit, investment, etc.)
    • Insurance documents
    • Medical records
    • Military records
    • Internet, cable, or satellite records
    • Vehicle records
    • Employment records

If you are able to do so, scan all three proof of residency documents into a single PDF. This makes registration much faster and is a mandatory step later in the season for All-Star registration.

 

All proofs of residency must show an address that matches the registration address and must EACH clearly show a date between August 31, 2011 and February 1, 2012.

 

After you fill out the registration form, you will be directed to the upload page.

If you do not upload these documents during online registration, you must submit these documents in-person at either of the two in-person registration days.
